Please have your card with you at the time of your visit. If we file your claim, you are responsible for only the co-pay at the time of the visit.
If we are not a provider for your insurance, if you are not insured or if we do not file with your insurance company, you may be responsible for the entire charge at the time of service.
Be sure to check with your insurance provider to ensure coverage before scheduling an appointment.

* Insurance plans typically allow a 30-day window from when your new baby is born to add them to your plan; if your baby is not added in that time you may be responsible for your full balance.
* If your insurance plan requires you to select a primary care provider, you may be responsible for any balance for the time of service unless you have selected a provider from our office.
